- Accident report (Certified copy),
- Family's civil registry statement,
- Death certificate and autopsy report, if available,
- Burial license,
- Certificate of inheritance.

In case of disability
- Accident report (Certified copy),
- Disability report specifying the disability percentage (from a general hospital),
- A letter indicating whether the person has received payment from the Social Security Institution or not.

In case of injury
- Accident report (Certified copy),
- Original copies of documents attesting treatment costs (invoices, prescriptions, receipts, drug barcodes),
- Epicrisis issued by the hospital,
- Bank account details.
- If deemed necessary, the Insurance Company may request supplementary/additional documents.
