Information required to be provided to the insurer during damage reporting is as follows:
1) Insurance Policy number
2) Name and/or Title of Insured/ Affected Party
3) Date of Damage
4) Damage Location
5) Damage Occurrence Means
6) Estimated Damage Amount
7) Mail address of the location where expertise service is to be provided, contact information/ contact person
to be called by Company official (phone, address etc.)

Please note the file number you shall be provided upon damage reporting.
This file number shall be used for all correspondence and communication related to your damage.
